To participate in intercollegiate athletics, student-athletes
MUST complete a Sports Medicine Profile. All requirements (with the exception of concussion testing) should be completed by
August 1.
Your Sports Medicine profile includes:
- SportsWareOnline (SWOL) profile
- Sports Medicine Health History & Physical Examination for Augustana College
- Proof of Sickle Cell Trait Test
- Medical Release from a Medical Provider (if applicable)
- NCAA Medical Exemption Documentation (if applicable)
- Concussion Testing (completed on campus after campus)
1. SportsWareOnline (SWOL)
SportsWareOnline (SWOL) is a secure, cloud-based electronic medical record (EMR) portal used by athletic trainers and student-athletes to manage injury tracking, medical clearance paperwork, insurance information, and treatment documentation.
New student-athletes will have access to the SWOL porta
l beginning the end of May. More information will be shared via email once the portal is available. Student-athletes will complete SWOL registration each year prior to NCAA participation.
Please follow these
step-by-step instructions for new student-athletes to assist in setting up your SWOL account.
The following documents can be uploaded to SWOL:
- History and physical form
- Injury or illness releases
- Sickle cell test results (first-year and transfer students only)
- NCAA exemption forms, if required
To upload documents in SWOL, go to
FORMS and click
ADD.
Important: HEIC photo files from iPhones are not accepted. Please convert files to JPEG, PNG or PDF before uploading.
2. Sports Medicine Health History & Physical Examination for Augustana College
Student-athletes must complete a health history and physical exam each year. Student-athletes are responsible for scheduling and covering any expenses associated with their physical exam. Yearly physical exams completed by the following medical professionals will be accepted:
- MD (Doctor of Medicine)
- DO (Doctor of Osteopathic Medicine)
- PA (Physician Assistant)
- NP (Nurse Practitioner)
Chiropractic physicals are not accepted.
The
Augustana College Sports Medicine Health History can be completed by you, then initialed and dated by your medical provider.
Please provide the editable PDF or a printed copy of the
Physical Examination for Augustana College to your medical provider at your physical exam.
3. Proof of Sickle Cell Trait Test
The NCAA requires all first-year and transfer student-athletes to show one-time proof of a sickle cell trait test. This should be done promptly, as it can take time to find results. If you are unable to access your results, have your medical provider run the test.
Domestic students can learn how to find their test results
here. Once you have your results, upload them to SWOL.
International students have two options for completing your required hemoglobin electrophoresis (Sickle Cell) test. You may have the test performed by a medical professional in your home country before departure, or we can provide a lab order for you once you arrive on campus. If you choose to test after arrival, please coordinate with your athletic trainer immediately to prevent delays in your athletic clearance. Please note that students are responsible for the cost of this testing.
4. Medical Release from a Medical Provider (
if applicable)
If you saw a medical provider (MD, DO, NP or PA) within the past year for injury, illness, surgery or physical therapy, you must obtain a medical release form that clears you for participation. Upload the release to SWOL or fax it to 309-794-8709.
5. NCAA Medical Exemption Documentation (
if applicable)
This
NCAA Medical Exemption Documentation for Augustana College is for those taking medications for ADHD and/or breathing issues possibly banned by the NCAA. More information on the NCAA Banned Substances can be found
here.
The prescribing practitioner must fill out any forms with you. Please upload the completed form and justification documentation to SWOL. The form can also be accessed in SWOL under
Notices And Handbooks.

6. Concussion Testing
All student-athletes will complete concussion testing when they arrive on campus.
Please download the
SWAY Medical app to your mobile device before arrival to campus, but don’t do anything else with it. You will receive an access code during your concussion testing time.
Tip: ensure your device is fully charged prior to your concussion testing time!
Fall sports concussion testing will take place during pre-season training.
Winter and spring sports testing date and time are TBD.
