Mandatory Pre-Participation Forms
Each student-athlete is required to have a physical examination prior to participation in any intercollegiate sport. Please download the correct forms below and then complete Athlete Connection. Fill paperwork out and send it back as soon as possible to Augustana College, Sports Medicine, 639 38th St, Rock Island, IL 61201.

Don’t forget to include an ENLARGED front back copy of your insurance card when you turn in your packet.
If you are taking medication for breathing issues or ADHD you must fill out the appropriate NCAA exemption form with your prescribing medical care provider.

The final decision on athletic participation status will remain the responsibility of Augustana's team physician upon thorough review of the athlete's medical information. Student athletes are responsible for scheduling and payment of their own physicals.
Student athletes will not be allowed to participate without completing the entire sports medicine packet. Make sure to make a copy of your completed packet

Sports medicine presentation
The Sports Medicine Presentation is mandatory for all student-athletes participating in NCAA intercollegiate sports at Augustana. Enrolling in the
Moodle course requires an Augustana email and password. First year/transfer student-athletes have to visit campus to register for classes and set up your Augustana email accounts before logging in. International first year/transfer students will usually set up your accounts when you arrive on campus in the fall.
To log into Moodle use the first half of your Augustana email- DO NOT add @augustana.edu- then your password. Once in Moodle, scroll down Course Categories to "Offices" and click. Scroll down until you see "Sports Medicine Preseason Presentation". Self-register for the course and follow the directions carefully. After viewing the concussion video, NCAA Drug Testing video and powerpoint presentation (ppt), students must take the Moodle quiz and get a 100%, otherwise the viewing will not count. By taking the quiz and passing you are confirming you viewed all of the required materials. Click
